Medical gas inspection

The other day a customer of mine contracted me to perform a medical gas inspection on an addition to an existing facility (small digestive center). Here is what I found...

2 PM arrived on time and spoke with the installers (who I had never met previously).  They asked me to come back in about a half hour.

The pictures I have uploaded cannot begin to show everything I found that was either incompetent or fraudulent.

Try to look at the pictures first before reading below. I believe this could be a good exercise....

BTW, I tried to rotate them, but the pictures still uploaded sideways-sorry.  Also, the picture of the pipeline may be difficult to see.  Don't spend too much time on that one.

    ______________ _____________ _________ _________ __________

The picture of the cylinder should be an easy one.  (At least 3 things wrong...)

The pictures of the pipeline may be difficult to see, but together show something that should not be done...

The document shows what was given as proof of NITC credentials.

I would like to allow time for comments on the pictures before I fully explain what I found.

    Hi Al,

Looks like it may be just the two of us on this one...

Falsified NITC docs ( you don't see Red in the expiration dates...plus you dont see a brazer expiration out 1-year plus...)

Cylinder contents label altered because it was not NF.

O2 and med vac lines interconnected (brazed) (bad picture)

Zone valve piped backwards (no picture)

    Fittings not O2 cleaned...

And so on, and so on,,,

No purge alarm is the second of two (with the defaced label being the first...)

Third is one of my personal pet peeves (freestanding cylinder with no restraint...)

    Also,

It was suggested to me that I was incorrect about the zone valve being piped backwards.

I was going to fire back that it is to be installed per mfg's instructions, and that the mfg would require left to right installation to locate the gauge on the downstream side, and have that gauge visible through the transparent openings in the otherwise opaque window...

As it turns out, this particular zone valve had dual ports and a large window (allowing clear view of the gauge in either orientation), and plainly states in the literature "May be plumbed left to right or right to left".

So, I was wrong on that initial post regarding the zone valve.  (Thanks to Al for bringing that up to me...)
