CMGV® Credentialed Medical Gas Verifiers and MGPHO members holding current ASSE 6030 credentials who are planning on taking the CMGV® Credential and Re-Credential examinations should review the following information for instructions on registering for and taking the exams.
The purpose of the Credentials Committee is to ensure that MGPHO members are successful at developing their knowledge and expertise in the field of medical gas verification. It is with this purpose in mind that the outline, matrix and review class are presented to our membership.
The 2015 CMGV® Credential and Re-Credential exams will be a major focus of this year’s annual educational conference in Kansas City, Missouri.
Re-Credential Examination (applies to current CMGV® verifiers only)
Location: Marriott Country Club Plaza, Kansas City, Missouri
Date: October 1, 2015
Time: 1:15 p.m. – 2:15 p.m. for review session
2:30 p.m. – 4:30 p.m. for written examination
Format: The written examination will be an open book test consisting of
100 questions. You must bring your own books to take this
exam.
Credential Examination (applies to new CMGV® candidates only)
NOTE: New CMGV® candidates will sit for their test equipment
review in the morning and take the written examination in
the afternoon.
Location: Marriott Country Club Plaza, Kansas City, Missouri
Date: October 1, 2015
Time: 6:30 a.m. to 8:00 a.m. for test equipment review (by
appointment only)
1:15 p.m. – 2:15 p.m. for review session
2:30 p.m. – 4:30 p.m. for the written examination
Format: A review of your test equipment will be conducted with a focus
on your knowledge of the equipment, and the proper care,
application, calibration and test results. You must have your
test equipment with you during this review. If you do not have
test equipment present, you will not be eligible to sit for the
written examination.
The written examination will be a closed book test consisting of
50 questions.

Application Process:
What do you need to do now? (applies to both Re-credential and new Credential candidates)
1. To be eligible to sit for the CMGV® Credential/Re-Credential Examination:
a. Have your MGPHO dues paid in full no later than September 1, 2015
b. Be a member in good standing by no later than September 1, 2015
2. Go to the MGPHO website and register and pay for the 14th Annual
Educational Conference as a Member w/CMGV® Exam option for a total cost
of $290.00
3. Download and complete the CMGV® Credential Application and provide the
following:
a. Current Certificate of Liability Insurance coverage of at least $1,000,000.00 per
occurrence
b. Copy of your current ASSE 6030 credential with an expiration date of October 4,
2015 or later
c. Submit any special needs you may have no later than September 1, 2015,
such as a language barrier or a physical limitation
d. A passport photo in either paper or software format (JPG required)
4. Submit your application in advance and as soon as possible. Applications will
be accepted as late as September 30, 2015.

Here is what you can expect from MGPHO:
1. Once we receive your application and you are qualified to sit for the exam,
MGPHO will send you a study guide and a matrix showing the breakdown
percentage of the exam regarding NFPA 99 sections covered in the
examination. The intent is for you to use for individual study of the NFPA 99,
2015 Code prior to taking the exam.
2. On October 1, 2015 MGPHO will conduct a review session of the Credential
Examination from 1:15 p.m. to 2:15 p.m.
a. The review will concentrate on the changes from 2012 edition to the
2015 edition along with the matrix breakdown of the exam
b. Review of the test rules, restrictions and question challenges will be
conducted at the end of the review session
